This one has several steps. First, your arm should be hanging down at your side, not held straight out as this will drastically shorten the measurement. Then the measurer starts at the tip of the end of your shoulder and measures to the elbow. Have your arm slightly bent for this, about 5 degrees or so. Note this first measurement. Then, measure from the elbow to the desired cuff length (either the actual wrist or to however far past the wrist you prefer your sleeves). Note this second measurement. Add the two measurements and you should have an accurate arm length suitable for garment construction.
